Duke Power Company McGuire Nuc1 car Station Response to Violations in Inspection Report 50-369/85-22 and 50-370/85-23 Violation 369/85-22-01, 370/85-23-01, Severity Level IV 10 CFR 20.101(a) requires that no licensee possess, use, or transfer licensed material in such a manner as to cause any individual in a restricted area to receive in any period of one calendar quarter from radioactive material and other sources of radiation, a total occupational dose in excess of 7.5 Rems to the skin of the whole body.
Contrary to the above, on June 5, 1985, licensed material was used in such a manner as to cause an individual in the Unit 1ReactorBuiljing,arestricted area, to receive an occupatie tal dose of 10.6 Rems to I cm of the skin of the whole body, resulting in a total occupational dose for the individual for the calendar quarter of 11.18 Rems to the skin of the whole body.

Admission or denial of the alleged violation:
l Duke Power agrees that the violation occurred as described in Licensee Event Report (LER) 369/85-21 and LER 369/85-21-01 (Revision 1).

Reasons for the violation:
On June 5,1985, a radioactove particle was discovered under the upper lef t arm of a technician involved in steam generator tube plugging who was exiting containment. The particle was removed and later analyzed.
Initial dose calculations indicated a dose of 10.6 Rem to the skin and 0.030 Rem to the I
whole body, which represents an overexposure to the skin for the quarter j
pursuant to 10 CFR 20.101a. A review of methodology and assumptions of the initial dose calculation has resulted in revised dose calculations which yield lower values for the dose which do not represent an overexposure.
Details of this event are described in LER 369/85-21-01.

Corrective steps which have been taken and the results achieved:
The particle was removed and later analyzed. The individual was removed from further activities involving radiation exposure for the remainder of the quarter. Further details are included in LER 369/85-21 and 369/85-21-01.
4.
Corrective steps which will be taken to avoid further violations:
i The following corrective actions were initiated after the original personnel contaminations:
Extensive frisking (80 man-hours per day) of all protective clothing.
Disposal of all cloth coveralls and hoods. New coveralls and hoods purchased. Prior to mass disposal only protective clothing used in high contamination jobs had been disposed of as radwaste.
Page 2 Increased the number of personnel frisking stations within the plant.
Frisking the inside of protective clothing by purchasing special (yellow) coveralls to be worn in very high contamination and high temperature areas.
More use of plastic and disposable protective clothing where possible.
Requisition of an automatic laundry monitor to eliminate technician error.

McGuire Nuclear Station is presently in full compliance.

a Page 3 Violation 369/85-22-02, 370/85-23-02, Severity Level IV 10 CFR 20,203(b) requires a licensee to post each radiation area with a conspicuous sign or signs bearing the radiation caution symbol and the words:
Caution - Radiation Area. A " radiation area" is defined, in 10 CFR 20.202(b)
(2), as any area, accessible to personnel, in which there exists radiation, originating in whole or in part within licensed material, at such levels that a major portion of the body could receive in any one hour a dose in excess of 5 millirem, or in any five consecutive days a dose in excess of 100 millirems.
Contrary to the above, on June 13, 1985, the licensee did not post the radiation area along the south wall of Room 637 on the 716' elevation that had dose rates of 72 millirem per hour at near contact with the interface area of the wall and floor and six miliirem per hour at 18 inches and at approximately waist height in the vicinity of the source.

Admission or denial of the alleged violation:
Duke Power agrees that the violation occurred as stated.
2.
Reasons for the violation:
The area in question is surveyed on a weekly routine and during radwaste events that have historically changed the radiation levels in the area.
The dose rate the previous and subsequent week was <2mr/hr. The event that caused the high radiation level was investigated and not determined.
3.
Corrective steps which have been taken and the results achieved:
The area was posted. Events surrounding the time period were investigated.
No cause could be determined. Radwaste personnel were reminded.to notify HP personnel when performing tasks that could change the radiation levels.
4.
Corrective steps which will be taken to avoid further violations:
This area will be monitored more closely and continue to be surveyed on a weekly routine and during known evolutions that change dose rates in the area.
5.
Date when full compliance will be achieved:
McGuire Nuclear Station is presently in full compliance.
