Respiratory Qualifications The purpose of this letter is to communicate corrective actions regarding the Emergency Plan Revision 96-1 error.
Please reference T.C. McMeekin letter dated September 5, 1996.
A problem investigation process (PIP)* report was initiated and a cause analysis performed.
The cause was human performance error involving inadequate self-checking. This error was reviewed with the individuals involved with emphasis on the need for a thorough administrative check of Emergency Plan revisions.
