Inspection 50-322/85-22
A special inspection was conducted by Mr. E. M. Kelly of this cffice during
the period April 10 - May 10, 1985, regarding allegations provided by a private
citizen to the New York State Consumer Protection Board which were, in turn,
presented to the NRC in a March 12, 1985 letter.
The allegations involve the
calibration of certain instrumentation and controls, as well as the training
and qualification of instrumentation technicians and supervisors. These were
characterized as 15 separate concerns by NRC Region I, and are listed in the
attachment to this report which was provided to you during this inspection.
The preliminary findings of this inspection are that two of the alleged con-
ditions, while restricted to non-safety related systems and equipment, were
substantiated.
These allegations were related to pressure switches in the

reheat steam system and level switches in radwaste laundry tanks. The

allegations involving tapping of indicators during calibration and a HPCI low
pressure switch were substantiated in part and require evaluation by your I&C

staff. Three of the allegations are issues previously identified and reported
to the NRC by your staff, and are either resolved or being followed as separate
NRC inspection items. Three of the allegations addressed the qualifications
and experience of unspecified technicians and supervisors involved in initial
instrumentation calibrations and surveillance; these were found to be unsub-
stantiated. Finally, a number of the allegations could not be assessed by
our initial inspection due to a lack of specific information.
For those allegations that were substantiated, we would like you to respond as
to their extent (i.e. isolated cases versus broader implications) and your
analysis of their root cause and any corrective action you may propose or have
previously applied towards preventing those types of errors from occurring in
otner systems. We would also like you to respond to us regarding evaluation of
calibration procedure SP 46.030.01 for possible incorporation of a procedurally
required downscale tap and the nominal set point and accuracy, as well as pro-
cedural verification of impulse line venting, for HpCI low suction pressure
switch PS-0216.
