NRC INSPECTION REPORT 50-456/96021; 50-457/96021
Dear Mr. Stanley:

On January 10,1997, the NRC completed an inspection at your Braidwood 1 & 2 reactor
facilities. The enclosed report presents the results of that inspection. Overall, we found
that plant operations were conducted safely.
.
We are concerned about a violation with eight examples of failing to follow procedures
demonstrated by operators, maintenance workers, radiation protection technicians, and
security guards that were, for the most part, identified by the inspectors. Procedure
adherence problems that have resulted in both cited and non-cited violations were
previously discussed in inspections Reports 96014, 96012, 96011, 96006, and 95017.
We are also concerned about two other violations. One violation was for failure to comply
with the American Society of Mechanical Engineers Code and promptly declare one train of
the Essential Service Water System inoperable when a through wallleak had been
identified. Sufficient quidance on this issue had been previously provided through NRC Generic Letters 91-18 and 90-05 to allow the Braidwood staff to understand what actions
needed to be taken. The other violation was for failure to take timely corrective action for
a number of carts and equipment that had been improperly secured to or in the immediate
vicinity of safety-related equipment. This issue required repeated identification by the NRC
to ensure appropriate corrective actions were taken.
These violations are cited in the enclosed Notice of Violation, and the circumstances

surrounding the vio?ations are described in detailin the enclosed report. Please note that
you are required tu respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.

Because of the number and diversity of examples in the Notice, in addition to your
response to the specific violation, provide an additional explanation of what steps you have
or are planning to take to assure that this will not become an adverse trend and continue
