NRC INTEGRATED INSPECTION REPORT 50-352/96-07,

50-353/96-07; AND NOTICE OF VIOLATION

On August 27 through October 21,1996, the NRC completed an inspection at your

-

Limerick 1 & 2 reactor facilities. The enclosed report presents the results of that inspec-

tion,

During the 8-week period covered by this inspection period, conduct of activities at the

Limerick 1 & 2 facilities was generally characterized by safe and conservative operations.

.

The inspectors noted that operators and maintenance personnel conducted activities very

<

well. Engineering support of activities was excellent. System managers were seen to be

very involved in activities for their systems, which resulted in the resolution of issues

quickly, and limited the time for systems to be in technical specification limiting conditions

for operation. Additionally, management appeared to be involved in making conservative

decisions and in supporting plant personnel in resolving issues. This resulted in, among

other things, reducing the non-outage corrective maintenance backlog from approximately

900 in June 1996, to approximately 600 in October 1996.

Based on the results of this inspection, the NRC has determined that a violation of NRC

,

requirements occurred. The violation is cited in the enclosed Notice of Violation (Notice)

I

and the circumstances surrounding it are described in the subject inspection report. A

temporary change was made to a transient response implementation plan procedure and

,

was not reviewed and approved by the appropriate manager designated by administrative

procedures within 14 days of implementation. Additionally, the inspector identified other

concerns with your procedure temporary change process. The process did not assure the

fidelity of temporary changes to programmatic controls for certain higher level procedures

listed as plant impact procedures. Also, your controls for maintaining designated

individuals with the authority to approve procedure changes were weak.

The NRC has concluded that information regarding the reason for the violation, the

corrective actions taken and planned to correct the violation and prevent its recurrence are

,

already adequately addressed on the docket in the subject inspection report. Therefore,

you are not required to respond to this letter unless the description therein does not
