Dear Mr. Denise:
The references provided a cortprehensive description of potential concerns with Cutler-Harmer Type E30 pushbutton switches.
This letter provides supplemental information requested by you concerning the results of field inspections and engineering evaluations of the i
specific switches in question.
Attached is a table which provides this information.
i A total of 457 safety related Cutler-Hamer Type E30 switches have been identified and inspected.
Of these, 294 could defeat an automatic safeguards actuation if the plunger /contactor " binds" in the extended position as described in the References.
During the initial inspection of the 457 safety-related switches only 5 were found in the stuck position.
The ongoing verification inspections have not identified any additional instances in which a plunger /contactor have
' bound" in the exterded position.
As part of the ongoing evaluation into the cause of the problem, KG&E J
relected several installed switches which have had no indicated 3
problem.
These switches have been renoved and sent to Cutler-Hammer
(
for their use in the overall evaluation of the problen.

LEG?D SWrKH This colum provides the switch identifiers which consists of three parts.
The first two alphabetic characters represent the plant system.
The abbreviation HS stands for handswitch, while HIS stands for hand indicating switch.
The third alphanumeric portion provides a unique identifer for a switch in a given system.
LOCATION This column indicates panel location nunbers if applicable.
Q This column indicates whether a specific switch is or is not safety-related.
A "Yes" entry identifies that switch as safety-related.
CONPACT TYPE This column indicates what type of contact blocks were present when the switches were initially visually inspected.
" Single" identifies single circuit contact blocks, while
" Double" identifies two circuit contact blocks.
PLUtER POSTI' ION This column indicates the plunger /contactorpositions present when the switches were initially visually inspected.
" Extend" identifies a plunger /contactor found in the
" bound" condition (i.e. other than normal).
SPAG This refers to the cor. tact block operating stack.
TH This refers to the upper operating stack of a switch.
HE This refers to the lower operating stack of a switch.
INHIBIT AU10mTIC SAFEGERRD ACTERTION A
"Yes" response in this colum indicates that a " bound" plunger /contactor could prevent an automatic safeguards actuation from being acconplished. A "No" response indicates that an automatic safeguards actuation could not be prevented from being accccplished.

DIFFICULT TO OBSERVE This colum gives 'an indication of operator awareness of. adverse plunger /contactor condition without visually inspecting the plunger /contactor.
