SUBJECT: NRC INSPECTION REPORT 50-416/96-17 AND NOTICE OF VIOLATION
Dear Mr. Hagan:

An NRC inspection was conducted September 29 through November 9,1996, at your
Grand Gulf Nuclear Station reactor f acility. The enclosed report presents the scope and
,
results of that inspection.
During the 6-week period covered by this inspection period, your conduct of activities at
the Grand Gulf facility was generally characterized by safety-conscious operations, sound .
,
!
engineering and maintenance practices, and careful radiological work controls. We are
concerned, however, about two violations of NRC requirements that were identified.
The first violation is of concern because operators failed to perform Technical Specification
i
surveillance requirements because of an incorrect information tag which had not been peer
i
reviewed for accuracy. The second violation is of concern because an electrical
,
maintenance supervisor failed to revise a work package when the work scope changed.
This failure to revise the work package resulted in an electrician receiving an electrical
i
shock while working on a motor control center that was powered' from a temporary source,
i

- The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violations are described in detailin the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the
i
l
enclosed Notice when preparing your response. The NRC will use your response, in part,
l
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
