Region I Inspection 50-309/85-19
This transmits the August 8-16, 1985 special inspection findings of Messrs.
C. Holden and K. Ferlic at the Maine Yankee Atomic Power Plant, Wiscasset, Maine.
The inspection was conducted to determine the circumstances surrounding the dis-
covery of mispositioned root valves for the steam generator pressure transmitters.
Those findings were based on observations of activities, interviews, and document

reviews, and have been discussed with Mr. Garrity of your staff.

Apparent violations of NRC requirements are cited in the enclosed inspection re-
port. We are considering these violations for appropriate enforcement action which
will be addressed under separate correspondence at a later date.
,
Our investigation into this event indicates a failure to administratively control
the position of the Steam Generator Pressure instrument root valves.
Generalized-
statements in the Hydrostatic Test procedure did not ensure that all root valves

that were positioned for the test were returned to their required position.
As
a result, the plant operated since the end of the last outage date, June 20, 1984,
with these root valves mispositioned.

We consider this event to be serious and have scheduled an Enforcement Conference

for September 9, 1985 to discuss this issue.

Your methods for administrative control over safety related valves.

The methods you intend to use to verify operability of safety related systems

and instrumentation.
What actions have been taken to ensure this or other safety systems have not

been inadvertently made inoperable.
Other protective functions available to provide backup protection for the in-

operable pressure trips and degree of protection provided.
