DEVICE TYPE: Vertical Teletherapy Unit l

DESCRIPTION:

The Keleket-Barnes Flexaray is a motor-driven floorstand teletherapy unit designed for stationary beam therapy. The unit can be ordered with a-yoke as an optional feature. The treatment distance can be varied, beginning at 30 cm (11.8"). The height of the source above the floor can be adjusted from 2'4" (71.1 cm) minimum to 6' (183 cm) maximum. The unit is 8' (244 cm) high and the distance between the source and the back of the unit is l approximately 5' (152.4 cm).

The unit normally uses a standard international teletherapy capsule and has been licensed for up to 4000 curies.

The unit may be supplied with a choice of two different heads.

The Mark II, Model D head, is rated for 1500 rhm source and the Mark II, Model D-1, is rated for 3500 rhm. The head consists of a spherical cast steel shell filled with lead. A tungsten alloy disc inside the head provides additional shielding in the "off" position for the Model D-1 head. The source is seated in a cavity in the solid tungsten alloy source wheel and held in place by a lock ring. The system is designed so that the source wheel l will be automatically returned to the "off" position by a return '

spring in the event of power failure.

The head will tilt forward 225* from the downward direction and back 45*. The yoke (optional) may rotate through an arc of 360*.

Beam orientation in the two planes can be limited by microswitches.

A pointer on a hand wheel and lights on the control panel and i inside the teletherapy room indicate the "on" and "off" conditions. The source wheel retracts to the "off" position by a return spring if the power fails or the door is opened, the unit must be reset to continue treatment. In an emergency, the indicating hand wheel at the rear of the head can be turned to return the source to the "off" position. The source cannot be

! moved to the "on" position by the hand wheel, however, because of l a one way clutch.

The device may be used by persons specifically licensed by the NRC or an Agreement State.  ;

1 SAFETY ANALYSIS

SUMMARY

LFE' originally registered this device with the intent to commercially distribute it. NRC concluded that the device was acceptable for licensing purposes. However, the devices were never distributed from LFE and, therefore, LFE has no information on the number of devices distributed. LFE does not intend to distribute the device in the future.

This letter is in reference to your letter dated August 6,1991, which requested that we transfer the registration certificates listed in column 1 of Enclosure 1.to inactive status. The certificates have been transferred to ,

inactive status as you have requested. Please note that transfer of the registration certificates to inactive status means the sources or devices included on the certificates may no longer be commercially distributed.

The registration numbers for the certificates have been changed. Enclosure 1 specifies the old registration number, the new registration number, and the model numbers. Please note that some old certificates have been combined into one new inactive certificate. In addition, two registration certificates have been deleted from the system since they were duplicates of other certificates.

Please review the registration certificates (copies enclosed in their entirety and notify us immediately if there are any errors or) omissions.
