Gentlemen:
This refers to the routine safety inspection conducted by Messrs.
A. L. Madison and A. D. Morrongf ello of this office on June 1 through
July 31, 1985, of activities at Quad Cities Nuclear Power Station, Units 1
and 2, authorized by NRC Operating Licenses No. DPR-29 and No. DPR-30, and
to the discussion of our findings with Mr. N. J. Kalivianakis at the
conclusion of the inspection.
The enclosed copy of our inspection report identifies areas examined during
the inspection. Within these areas, the inspection consisted of a selective
examination of procedures and representative records, observations, and
. interviews with personnel.
During this inspection, certain of your activities appeared to be in violation
of NRC requirements, as specified in the enclosed Appendix.
A written response is required.
In addition to the violations, we are concerned with the ongoing problems
experienced with the refueling floor monitor and the attendant safety system
challenges associated with the automatic start of the Standby Gas Treatment
System. Normal corrective maintenance has not been effective in preventing
these spurious actuations, and additional engineering effort appears necessary.
Therefore, we are requesting that your response also address what actions you
are taking to eliminate further spurious actuations and the schedule for
completion of these actions.
