SUBJECT: NRC INSPECTION REPORT 50-483/96-10 AND NOTICE OF VIOLATION

An NRC inspection of steam generator program activities and material condition was
i
conducted October 21 through November 22,1996, at your Callaway Plant reactor facility,
!
with exit meetings conducted on October 25, and at the conclusion of the onsite
inspection. The inspection was continued by inoffice review, with a final telephonic exit
,
meeting conducted on December 9,1996, with Messrs. T. Hermann, T. Pettus, and
M. Reidmeyer of your staff. The enclosed report presents the scope and results of that
inspection.
Additionalinformation was received by facsimile from your staff pertaining to two areas of

identified concern. The information in Attachment 3 to the inspection report was received
I
on December 17,1996, and pertains to the absence of procedural guidance for setup
l
parameters to be used in analysis of plus point probe eddy current data. Attachment 4 to
i
the inspection report contains information that was received on November 26 and
l
December 19,1996,in regard to monitoring of essential variables during laser welding of
}
Overall, the results of the inspection indicate that you have developed comprehensive
1
program requirements and initiatives for detection and mitigation of degradation in steam
generator tubing. We consider your adoption of new eddy current examination technology
in the last refueling outage for examination of hot-leg side expansion transitions to be both
proactive and indicative of management support for steam generator tube integrity
initiatives. One violation was identified, however, pertaining to the f ailure to define the
applicable phase rotation and span settings for analysis of plus point probe eddy current
data. This issue is of concern since we believe that it, together with other information
obtained during the inspection, point to a current lack of organizational knowledge

regarding this examination discipline. We view this status as adversely affecting your
j
ability to perform effective oversight of eddy current contractor performance.

The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding this violation is described in detail in the enclosed report. Please note that you
are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,'
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
One unresolved item was identified concerning monitoring of essential variables during
laser welding of steam generator sleeves. This matter is discussed in Section M1.2 of the
enclosed inspection report and in Section 5.2 of Attachment 2 to the inspection report.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,
its enclosure (s), and your response will be placed in the NRC Public Document Room
(PDR). To the extent possible, your response should not include any personal privacy,
proprietary, or safeguards information so that it can be placed in the PDR without
redaction.
