Annual Report of Safety and Relief Valve Failures and Challenges (Required by Technical Specification 6.7.A.6 and NUREG 0737, item II.K.3.3). This report is provided as Attachment B.

This letter contains no new NRC commitments, nor does it modify any prior commitments.

ANNUAL REPORT OF MAIN STEAM SAFETY / RELIEF VALVE FAILURES AND CHALLENGES i

Failures 12/08/96 The plant was shutdown and the topworks (pilot and second stage assembly) associated with main steam safety relief valve RV-2-71D was changed out due to excessive pilot or second stage seat leakage.

Challenaes i

05/20/96 Following the 1996 refueling outage, all eight main steam safety relief l

valves (SRVs) were manually opened at reactor pressure of approximately 150 psig in accordance with a surveillance test performed during plant startup.

-l 05/20/96 Following correction of two SRV discharge line pressure sensing instrument problems which were discovered during performance of the above surveillance test, SRVs RV-2-71E and RV-2-71G were manually opened a second time later the same day in accordance with this same surveillance test. These SRVs were opened at a reactor pressure of approximately 150 psig.

12/08/96 SRV RV-2-71D was manually opened at a reactor pressure of approximately 150 psig in accordance with a surveillance test performed during the plant startup that followed changeout of its topworks.
