4.0 DEFINITIONS 4.1 Licensed senior operator (LSO) - An individual licensed by the NRC to manipulate the controls of a facility and to direct the activities of licensed operators.

Page 2 of 16 5.0 RESPONSIBILITIES 5.1 The General Superintendent Plant Operations SHALL be responsible for establishing shift schedules, requirements and personnel assignments.

5.2 The Shift Manager is responsible for the direction and coordination of the activities of the Shift Supervisors. The Shift Manager may direct the activities oflicensed operators or manipulate the controls which directly affect reactivity or reactor power level. The Shift Manager should be present in the Control room during plant transients but should maintain a hands-off-the-controls role.

l Overall decision authority on site during an accident is the responsibility of the Shift Manager. This decision authority may be transferred to an individual with an active l

SRO license.

1 5.3 The Shift Supervisor SHALL have the primary management responsibility for safe l

operation of the unit. The Shift Supervisor's highest priority SHALL be to maintain the broadest perspective of operational conditions affecting the safety of the unit.

The Shift Supervisor directs the activities of licensed operators and may manipulate the controls which directly affect reactivity or reactor power level. The Shift Supervisor should be present in the Control room during plant transients but should maintain a hands-off-the-controls role.

5.4 WHEN the Shift Supervisor is absent from the Control Room, THEN the other unit Shift Supervisor, the Shift Manager, OB an individual with an active SRO license SHALL be designated to assume the Control Room command function AND direct the activities of of licensed operators.

5.5 All on-duty personnel must be aware of the status of the portions of the plant and equ pment assicciated with their assigned area of responsibility. They must be alert and attentive to the equipment located within their areas until properly relieved.

5.6 On duty Contro: Room personnel SHALL ensure that the minimum staffing requirements for the Control Room are met at all times.

5.7 The Shift Supervisor or Control Room operators SHALL perform whatever proper actions are required to limit the operation of the plant or shut down the plant in the event of unusual or abnormal operations conditions. When analyzing plant operation conditions, the operators are responsible for believing instrument indications until the indications are proven incorrect.

6.0 B_EQUIREMENTS AND RECOMMENDATIONS 6.1 Minimum Shift Mannin_g 6.1.1 Each on duty crew should be composed of:

l q a. 1 Shift Manager (SRO)

b. 2 Shift Supervisors (SRO's)
c. 4 NLPERO/NPERO's (SRO or RO)
d. 6 Assistant Plant Equipment Operators or Plant Attendants (NLO or LO)

This DOES NOT include the licensed senior operator, or Senior Reactor l

Operator Limited to Fuel Handling, supervising refueling operations.

6.1.2 Minimum shift crew composition SHALL meet the requirements of Table 1, l

Minimum Shift Staffing.

l 1 6.1.3 There SHALL be at least one licensed operator within the Control Room I boundary, shown on Figure 1, for each reactor containing fuei.

6.1.4 There SHALL be at least one licensed senior operator within the Control Room boundary, shown on Figure 2, when either unit is in MODE 1,2,3, QB 4 (above cold shutdown).

6.1.5 it is the duty of the operators in Steps 6.1.3.A_1LQ 6.1.4 to ensure:

a. RCPs are stopped in accordance with the Emergency Operating Pro'c edures.
b. IE annunciator 47023-0603, Seism.; ' vent is actuated AND level is dscreasing in the Safeguards Cooling //ater Pump Bay (levelinstruments 41017,41011 QB 41503), THEN_ AB-3, Earthquakes Attachment A is immediately implemented.
c. AFW pump discharge pressure is maintained 2900 psig when an AFW pump is running.

6.1.6 The Shift Supervisor may assign additional licensed operators to manipulate the controls during critical evolutions or plant transient conditions. Refer to SWi-O-1.

SWI O-2 SWI SHIFT ORGANIZATION, OPERATION & TURNOVER REV: 35 Page 4 of 16 6.1.7 A fire brigade of at least five members SHALL be maintained on site at all times. The fire brigade SHALL NOT include the six members of the shift organization required for safe shutdown of the reactors.

6.1.8 An operator or Shift Chemist trained and qualified in radiation protection procedures SHALL be on site at all times when there is fuel on site.

6.2 Crew Organization ,

6.2.1 There SHALL be one senior licensed Shift Manager on duty at all times.

The Shift Manager SHALL be responsible for supervising activities affecting operation of the plant as a whole and SHALL havo the ultimate authority and responsibility during the shift.

l The Shift Manager's duties include the following.

a. Putting into affect the instructions received from the General Superintendent l

Plant Operations or designee. '

b. Ensuring that operations and turnovers are performed in conformance with these instructions.
c. Ensuring that the shift meets the staffing requirements of the Technical Specifications.
d. Performing Pre- and Port review of Work Order packages. Attend the l Daily / Weekly Work Planning meeting to review work planning and avoid operational conflicts.
e. Providing guidance on interpretation of the Technical Specifications and contacting the Operations Committee for resolution when required.
f. Ensuring the equipment control program (5AWI 3.10.0) is effectively implemented and recommending any changes to the program owner.
g. Ensuring the effectiveness of the Plant Security forces by coordinating unusual activities or circumstances with the Security staff.
h. Initiation of company notifications as required by 5AWI 3.6.4, Intra-NSP Notifications Regarding Plant Events or Conditions.

i. In an abnormal or emergency situation, the Shift Manager has overall responsibility for Operations' response to the event. The Shift Manager also provides technical expertise to the operating crew through the Unit Shift Supervisor.

When the E-Plan is implemented the Shift Manager is the primary individual to assume duties of the interim Emergency Director. lE conditions of the plant are such that the Shift Manager's full attention must be diverted from the I E-Plan duties, THEN the Shift Manager may delegate E-Plan implementation j to the unaffected Shift Supervisor.

j. Remaining within 10 minutes of the Control Room. To ensure thi.s requirement can be complied with the Shift Manager SHALL ensure that: l
1. Personal communication equipment (pager) is operable.
2. Shift Supervisors are aware of Shift Manager's location if outside the protected area.
3. Shift Manager SHALL NOT leave the site while on duty (PITC is included in site). l 6.2.2 There SHALL be one senior licensed Shift Supe visor on duty at all times for each reactor that contains fuel. Each Shift Supervisor on duty SHALL be assigned to a specific unit. .

! Each duty Shift Supervisor SHALL be responsible for supervising activities affecting the operation of the assigned unit.

The Shift Supervisor's duties include, but are not limited to the following:

a. Putting into effect the instructions received from the Shift Manager.
b. Reviewing surveillance procedures for proper performance and compliance with' Technical Specifications.
c. Ensuring that operations are carried out with maximum efficiency with due regard for safety and reliability and applicable regulations.
d. Reviewing logs for completeness and clarity of entries.

e. Conducting a tour of the plant on back shifts and weekends. (The total plant tour may be divided among the available SS's.)

These tours SHALL be logged in the Operations Log when completed.

It is not intended that each SS cover all areas of the plant during each shift.

Each SS should dedicate whatever amount of time they feel is warranted

)

and in whatever area they feel needs attention. Ideally, all areas of the plant will be covered between the two SS's during the course of a weekend or backshift run.

It is desirable to occasionally have other Control Room personnel participate in the outplant tours. This will allow for input from another perspective.

LPO's and RO's should be encouraged to periodically perform these tours.

Items and areas of concern during tours should include review of handwritten logs, communication usage, STAR concept, safety, human performance, housekeeping, equipment condition, radiation protection, environmental protection, procedures, and other areas of operationalinterest. .nese items should be observed on both a broad and narrow scope, i.e., safety should be 'l looked at all the way from a site, plant, system, component or process to l individual safety gear. i

f. The Shift Supervisor SHALL be responsible for maintair ng control of personnel entering the Control Room. Access should t ;imited to personnel )

on official business. He SHALL direct any personnel' ieave the Control Room when their presence may interfere with opers i. If necessary, the Shift Supervisor may lock the entrance to the Contrc doom to control access. This is accomplished by calling the guard hcuse and having the Control Room Door card readers removed from service.

g. Ths Shift Supervisors should generally use the Shift Supervisor's off;ce and the Control Room as their base location for directing plant operations; however, they may be anywhere in the plant where they believe their attention should be directed. The SS SHALL remain in the Control Room at all times during an accident situation on their unit to direct activ; ties of Control Room operators, unless properly relieved.
h. Notify the Shift Manager of off normal events or conditions.

6.2.3 The Lead Plant Equipment and Reactor Operator AN_Q the Plant Equipment and l

that the routine p. ant operations and requirements of the assigned unit are I carried out in a tiinely and efficient manner. These requirements include, but are l not limited to the 1ollowing:

a. Ensuring abncrmal AND emergency procedures are implemented when the Shift Supervisor is absent from the Control Room. -

b. Ensuring complete and proper log keeping.
c. Ensuring completion of scheduled surveillance procedures.
d. Implementing the Shift Supervisors instructions.
e. Notifying the Shift Supervisor of any abnormal plant or system response.
f. Proper response to and acknowledgement of all alarms. Significant alarms l are to be logged in the associated Reactor Log. l 1
g. Isolation and tagging or equipment to be " Held" or " Secured".
h. Conducting a tour of the plant if requested by the Shift Supervisor.
i. Reviewing all control board indications and computer printouts once per hour for unexplained changes or inconsistencies. The completion of this review, l with abnormalities discovered and required operator action, SHALL be recorded in the appropriate log. Abnormalities SHALL also be reported to the Shift Supervisor.
j. The Control Room operators should direct the plant operators to areas in the pl9nt which need attention.
k. Granting permission to non-Operations personnel for entry into the control area of the Control Room.

6.2.4 Shared Eculoment '

During routine plant operations, Control Room operator responsibilities for shared equipment are usually:

the Control Room, a verbal acknowledgement from the remaining operator who is required to support minimum Control Room staffing SHALL be received. This will preclude any inadvertent reduction in Control Room staffing below Technical Specification requirements.

6.2.6 The Assistant Plant Equipment Operator and the Plant Attendant should normally be in the plant making rounds to observe operating equipment, recording i operating data, and conducting routine operations, including radwaste and plant makeup system operations. l 1

The Control Room operator responsible for the unit should be notified promptly of all changes in plant status, abnormalities, any work taking place that could affect operating capability, or difficulties encountered in performing assigned tasks.

Each of these operators should normally be assigned to one of three general areas (1) the controlled area (" Auxiliary Building Operator"), (2) Unit 1 (" Balance of Plarit Operator") and, (3) Unit 2 (" Balance of Plant Operator"). The operator should stay in the assigned area unless directed elsewhere. In order to maintain the broadest possible plant familiarity each individual operator should be assigned, by the Shift Supervisor as Auxiliary Building operator and as balance of plant operator in a regular rotation.

SHIFT ORGANIZATION, SWI O-2 ST I OPERATION & TURNOVER REV: 35 Page 9 of 16 6.3 Shift Turnover 6.3.1 Shift change SHALL normally take place at the assigned watch station (SM Office, SS Office, Control Room, Auxiliary Building Operators Station, Unit 1 Turbine Building Operators Station, and Unit 2 Turbine Building Operators Station).

6.3.2 The outgoing shift operators SHALL continue with their duties until properly relieved by an operator of equal or higher classification unless otherwise directed by the Shift Supervisor.

l 6.3.3 The outgoing Shift Operator SHALL continue to hold the responsibilities of the position until" Status of the Day"information has been transmitted to the ,

incoming operator to the point that the incoming operator accepts the l responsibilities of the position. The incoming operator SHALL signify this acceptance by signing the Shift Turnover Log.

The information to be passed on to the incoming operator SHALL include, but not be limited to, the following:  ;

a. Current plant and system status. l
b. Review all OOS equipment.
c. Surveillance procedures outstanding.
d. Maintenance in progress or planned. l 1
e. Plant operation evolutions in progress or planned. l
f. Technical Specification LCO's - specify the time and date by which operability must be restored.
g. SigNificant events or alarms during last shift.
h. New operational or administrative procedures or instructions.
i. During outages, Shutdown Safety Assessment status.
j. All personnel SHALL use the Shift Turnover Log for this purpose.

As soon as possible after assuming the duty, the operator SHALL observe all indications and operating equipment in assigned areas to ensure normal operation and review the handwritten logs for events and equipment status changes that have taken place since last assigned to that watch station.

E middle of shift turnover is to a duty operator who attended the shift briefing,

  • THEiN turnover should consist of a review of the events that have occurred since the shift briefing.  ;

E middle of shift turnover is to a non-duty crew operator, THEN turnover SHALL be completed per section 6.3.3.

In emergencies, the outgoing operator may leave the duty assignment, with the I affected unit Shift Supervisor approval, before the incoming operator is on site I and has accepted the duty. The Shift Supervisor SHALL make a log entry in  !

the Operations Log noting the time the outgoing operator was permitted to leave I the duty assignment and another log entry noting the time when the incoming l i operator has accepted the duty. In this situation, the incoming operator SHALL receive a shift turnover from an appropriate duty operator on the affected unit.

. l 6.4 Shift and Event Briefinas 6.4.1 Shift Briefing

a. Shift briefings should be held with available shift operators, chemist, and the

' l SEC at 0630 and 1830 in the Control Room.  ;

l l b. Available maintenance supervisors and scheduling personnel should also )

attend the 0630 weekday briefings.

c. Shift personnel unable to attend the briefing should be updated by other duty personnel in attendance.
d. The purpose of the briefing is to inform plant personne! of plant status, evolutions in progress or planned, and tasks scheduled during the shift.
e. Noteworthy events /near misses / changes information should be conveyed by the duty Shift Manager to all crews, since last on duty, using PINGP 1224, {

f. The briefing should be conducted by the Shift Supervisors with additional information solicited from each duty station. Topics to be considered should include, but are not limited to, the following:
1. Current plant and system status.
2. LCO Action Statements in effect.
3. Significant equipment out-of-service.
4. Planned surveillance and maintenance activities and upcoming evolutions.
5. Significant recent Pl or industry events.
6. Significant T-Mods, Special Orders, Cat 1 openings, and bypasses in effect.
7. New operational / administrative procedures or instructions.

8. Work order prioritizations (0630 weekday briefing). I
9. Fire Detection / Protection / Brigade concerns.
10. Security related status / concerns.
11. Plant chemistry status / concerns.
12. Use of operators' tools for safety and error reduction:

a) Pre-job briefings b) Communications triangle '

,c) Procedure usage d) STAR concept e) Maintain a questioning attitude

13. Operator work-arounds.

a. Event briefings should be held with available Control Room operators during ongoing AOP or EOP events. Outplant operators may also be requested to participate.

b. Control Room operators unable to participate in the briefing should be updated by other participating Control Room personnel.
c. Outplant operators should be updated on plant status by attending the briefing, attending an OSC briefing, or through periodic contact with Control Room personnel.
d. Event briefings should be held at periodic, convenient intervals, as determined by the Shift Supervisor. Any member of the Control Room team may request a briefing be held. Convenient times to hold briefings may be during procedure hold points, procedure loops, or procedure transitions.
e. The briefings should be conducted by the Shift Supervisor or Shift Manager with additionalinformation solicited from each duty station. Topics to be covered should include, but are not limited to, the following:
1. Summary of plant conditions.
2. Procedures / activities in progress.
3. Current and potential equipment or operational concerns.
4. Expected progression of evolutions.
5. Operating limits at which contingency actions will be initiated,
f. Evolutions which meet the Pre-Job Briefing requirements of SAWI 3.2.4, Conduct of Work, SHALL be conducted IAW SAWI 3.2.4.

l Page 13 of 16 6.5 Supervision of Trainees 6.5.1 Non-licensed individuals may operate controls that directly affect reactivity, l provided that:

a. They are under direction and in the presence of a licensed operator.

AND l 1

b. They are assigned to a license training program. (Alicense training program i is defined as a training program leading to examination by the NRC.) l AND
c. They have demonstrated system knowledge by either successful completion of P8120L, Ucensed Operator Systems Training, or the knowledge portion of l the Qualification Standard for the associated system is signed off. l The degree of supervision required is dependent upon the training and l experience of the ncn-licensed individual, but, close supervision is required of all trainee operations to ensure they are performed properly.

To assure uniform implementation of this policy,it is necessary to delineate which controls directly affect reactivity. The activities that fallinto this category are:

Manual rod movements l Manual control of feedwater regulating valves Baron additions or dilutions Operation of Auxiliary Feedwater that feeds the Steam Generator while the reactor is critical Operation of the turbine control system which causes a change in steam flow 4

6.5.2 In order to promote quality training and ensure proper cupervision of trainees, normally only one trainee per unit will be permitted. This requirement may be s

relaxed at the discretion of the Shift Supervisor.

6.5.3 Regardless of the competency of the trainee, the licensed operators in the Control Room are responsible for all manipulations and operator actions.

Therefore, all operations performed by trainees SHALL be observed by a licensed operator and verified correct prior to initiation and after completion of the manipulation.

Shift crew composition may be one less than the minimum requirements for a period of time not to exceed two hours in order to accommodate an unexpected absence of one duty shift crew member provided immediate action is taken to restore the shift crew composition to within the minimum requirements specified.
