SUBJECT: NRC INSPECTION REPORT 50-313/96-06:50-368/96-06 AND NOTICE
OF VIOLATION
Dear Mr. Hutchinson:
An NRC inspection was conducted August 18 through September 28,1996, at your
Arkansas Nuclear One, Units 1 and 2 reactor f acilities. The enclosed report presents the

scope and results of that inspection.

During the 6-week period covered by this inspection report, your conduct of activities was
j
generally characterized by safety conscious operations, sound engineering, maintenance
activitias, and careful radiological controls. Two Notices of Violation of NRC requirements
were identified. The first violation is of concern since the introduction of service air into
the reactor coolant system at reduced inventory caused an erroneous level and almost
caused operators to remove water inventory from the reactor coolant system. The second
violation involves a hydrogen burn in the pressurizer relief valve tailpipes due to welding

activities. This is of concern because programmatic requirements which could have
prevented the event were not implemented. These violations were considered for
enforcement discretion; however, they are being cited to emphasize the importance of
controlling and coordinating work activities during sensitive outage periods.

These violations are cited in the enclosed Notice of Violation (Notice) and the
l
circumstances surrounding the violations are described in detailin the enclosed report.
Please note that you are required to respond to this letter and should follow the
instructions specified in the enclosed Notice when preparing your response. The NRC will
use your response, in part, to ' determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,
its enclosure (s), and your response will be placed in the NRC Public Document
Room (PDR). To the extent possible, your response should not include any personal
privacy, proprietary, or safeguards information so that it can be placed in the PDR without
