B and decontaminated, repaired, or disposed of in accordance with Commission W regulations. The report shall be filed within 5 days of the date the leak test !E.l B result is known with the U.S. Nuclear Regulatory Commission, Region IV, 611 l M Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, ATTN: Director, Division A .of Radiation Safety and Safeguards. The report shall specify the source l gi g involved, the test results, and corrective action taken. gl 3

Tests for leakage and/or contamination shall be performed by the licensee or by g g other persons specifically licensed by the Commission or an Agreement State to gi g perform such services. gl M gl y 13.
