RITORT NUMBER:

SD 414/85-09 REPORT DATE:

July 12, 1985 FN'TT. TTY: Catawba Nuclear Station, Unit 2 IDENIIFICATION OF DPPICII?JCY:

'Ihe adhesive used to affix the tretal narreplates to the control power trans-formers (CPr's) enployed in the 600VAC motor control centers could lose its ability to maintain the namplates securely affixed which could result in naaeplates falling off.

INITIAL REIORT:

Ch June 28, 1985, Mr C W Burger of NRC Pcgion II, Atlanta, Coorgia, was noti-fied by telephone of the deficiency by Mr W H Jourdan, Mr J L Crenshaw and Mr T L Utterback of Duke Power Cmpany, Charlotte, North Carolina.

SUPPLIER AND/OR GNPOt&NP:

Nelson Electric Capany of Tulsa, Oklahma nnnufacturered and supplied to Duke Power Capany the 600VAC motor control center ass mbly. %e control power transfourcrs which are installed in the noter control center asserrblies were nunufa itured by IIEVI-DUIY Electric, a unit of General Signal of Golds-boro, NC.

DIECRIPPION OF DEFICII7JCY:

The control power transform 2rs (CPT's) which w2re supplied to Nelson Electric by IIEVI-DUIY Electric and which w2re installed in the 600VAC notor control centers for Duke Power Cmpany, Catawba Nuclear Station, unit 2 have netal nameplates affixed with adhesive. We adhesive could lose its ability to traintain the nanoplates securely af fixed to the CPr's with the result that sme naneplates could fall frcxn the CPr's.

The notor control center ccrrpartrents which sploy CPT's are of various size and function resulting in different locations for the nounting of CPr's.

%is results in several rossibilities of potential failures which include unintentional operation of a cnrpartnrnt function or the prevention from intentional functionity;. 01e rossible exanple of a specific failure node is a naneplate in a ruversing starter compartant used to control a notor operated valve shorts the auxiliary contacts of a contactor. %is would result in clyclic o[nration of the valve.

ANAINSIS OF SAITlY IMPI.RWPIONS; As of the date of thss report, the nchedulul date for fuel loading at Catawin Nuclear Station unit 2 is January 1,

1986. Until fuel is actually loadal there is no effect on nafe g nration of the plant.

If, however, the plant was in an operational status, the only safety inpli-cation would result fran the nameplates falling fran the CPT's in two com-partments which perfona related safety functions in opposite safety trains.

h possibility of this occurrence is considered to be negligible.

CDRRECPIVE ACPION:

To eliminate all possibility of failure of a notor control center compartnrent due to a fallen nannplate, it is planned to renove all netal CPT naneplates which are affixe,I with adhesive fran the compartments.

h renoval of nutal CPT nameplates fran all Catawba Nuclear Station unit 2 notor aantrol center canpartnents is scheduled for completion prior to January 1, 1986.
