Appendix B NOTICE OF DEVIATION Ohio State University Docket No. 50-150 As a result of the inspection conducted on August 26-28, 1985, it appears that certain of your activities deviate from commitments made to the NRC by the Ohio State University.
Deviation The Hazards Summary Report for the Nuclear Reactor Laboratory, Chanter 2, revised on July 3,1985, states in Section 2.2 that written penedures, reviewed and approved by a licensed Senior Reactor Operato. , shall be in effect and followed.
Contrary to the above, on two occasions, Procedure IM-0' , Use of the RY-4 Jumper, was not followed in documenting the use and aprroyal of the installa-tion and removal of the RY-4 Jumper and on one occasio', the documentation of SR0 notification of a high radiation smear per Procedure RS-08, NRL Smear Survey, was not followed.
