Gentlemen:
This refers to the routine safety inspection conducted by Mr. R. A. Westberg of
this office on September 16 through October 4,1985, of activities at the
La Crosse Boiling Water Reactor authorized by NRC Operating License No. DPR-45 and
to the discussion of our findings with Mr. G. S. Boyd and others of your staff at
the conclusion of the inspection.
The enclosed copy of our inspection report identifies areas examined during
the inspection. Within these areas, the inspection consisted of a selective
examination of procedures and representative records, observations, and interviews
with personnel.
Although no violations of NRC requirements were identified during the course of
this inspection, an unresolved item was identified relative to the thoroughness
of auditing the maintenance work request system. Based on this item and a
cursory review of audits in other disciplines, we are concerned that audits, in
general, may not be of sufficient depth. Because of the importance that se place
on audits as a means of assuring quality, we request that you respond to this
issue and specifically address the actions that you have taken or plan to take to
assure that audits are adequately comprehensive in all areas.
In accordance with 10 CFR 2.790 of the Coninission's regulations, a copy of this
letter and the enclosed inspection report will be placed in the NRC's Public
Document Room.
