NRC INSPECTION REPORT #85-35 NOTICE OF VIOLATION HOPE CREEK GENERATING STATION Your letter-dated September 5, 1985,. transmitted the above referenced Inspection Report which-contained a Notice of Violation citing one (1) item of nonccmpliance concerning l'

our Preoperational Test Program.

The following response is i

FSAR Section 14.2.12.1.48, Fuel Handling and Vessel Servicing Equipment, and GE Preoperational Test Speci-fication for Hope Creek (No-A4102300-32(1)-3: 22A2271AZ, J

Revision 2) state that all interlocks associated with the refueling and service platforms will be verified and

included as acceptance criteria in the preoperation test (PTP).

Contrary to the above, approved PTP KE-1, Fuel Handling and 4

Vessel-Servicing Equipment, Revision 0, did not include steps to properly verify a refueling interlock that prevents moving the refueling platform over the core in the Startup 4'

mode with all rods in and.all hoists unloaded..

FSAR Section 14.2.12.1.54, Amendment 11, Remote Shutdown Panel, states that all valves and controls associated with i

the remote shutdown panel will be tested and included as f

l test acceptance criteria.

Contrary to the above, valve LEG-HV-2520B, SACS cooling I

inlet to RHR pump B seals and motor bearings, was not j

included for testing in the PTP SV-1, Revision 0, Remote l

Shutdown Panel.

Corrective Steps Taken and Results Achieved As indicated in Inspection Report No. 85-35, Change Notices (CN) or On-The-Spot changes (OTS) were initiated to correct the items identified in the respective Preoperational Test Procedures (PTP's).

Corrective Steps Taken to Preclude Recurrence i

1.

The Hope Creek Test Review Board (TRB) was established to provide a highly detailed review of preoperational test procedures.

In this case, a mistake was made by all reviewers.

As a result of this violation, the Startup Manager has provided personalized training for the TRB l

and reemphasized the requirements that each PTP is a quality product and that all test commitments shall be fully tested and properly documented.
