NRC INTEGRATED INSPECTION REPORT NOS. 50-325/96 16, 50-324/96 16

NOTICE OF VIOLATION

Dear Mr. Campbell:

On December 7,1996, the NRC completed an inspection at your Brunswick reactor

facility. The enclosed report presents the results of that inspection.

During the six weeks covered by this inspection period, our ins)ectors found

that your staff generally took a safety conscious approach to t1e activities

conducted at the Brunswick plant. We are concerned, however, about two

violations of NRC requirements that were identified. The first violation was

for three exam)les of procedural requirements that were not followed resulting

in a loss of slutdown cooling. This is the third loss of shutdown cooling

event that has occurred in three years. We are concerned about your lack of

rigorous shutdown risk management which permitted scheduling of work

activities on a safety system while the system was needed to remove decay

heat. Additional management attention is needed to properly control these

activities and prevent recurrence of another event. Although this violation

has been categorized at Severity Level IV, you should be aware that in the

future such failures regarding loss of shutdown cooling could result in

escalated enforcement and/or civil penalties. The second violation was for

failure to have a radiological control procedure consistent with the

requirements of federal regulations. We are concerned because the restrictive

dose limitations and monitoring requirements for declared pregnant women were

not correctly implemented.

These violations are cited in the enclosed Notice of Violation and the

circumstances surrounding the violations are described in detail in the

enclosed report.

Please note that you are required to respond to this letter

and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to

determine whether further enforcement action is necessary to ensure compliance

with regulatory requirements.
