SUBJECT: NRC INSPECTION REPORT 50 d58/96-016 AND NOTICE OF VIOLATION
Dear Mr. McGaha:

An NRC inspection was conducted November 3 through December 14,1996, at your River
Bend Station reactor f acility. The enclosed report presents the scope and results of that
inspection.
,
During the 6-week period covered by this inspection period, your conduct of activities at
the River Bend Station facility was generally characterized by safety-conscious operations,
sound engineering and maintenance practices, and good radiation protection support. We
are concerned, however, about three violations of NRC requirements that were identified.
The first violation identified by the inspectors involved a system operating procedure for
the hydrogen control system that f ailed to include breakers that, if mispositioned, would
render all or portions of the hydrogen igniters inoperable. Of particular concern was that
personnel introduced the error during the procedure upgrade process and verification and

validation f ailed to identify the error.

The second violation, identified by the inspectors, was for the inadvertent entry into
Technical Specification 3.0.3 for a short duration for inoperable high and low pressure core
j
sprays due to a failure to properly implement your Safety Function Determination Program
as required by Technical Specifications.

The third violation was identified by the inspectors during review of a corrective action
document because your staff failed to properly implement the requirements of ASME Boiler
j
and Pressure Vessel Code Section XI,1980 Edition through Winter 1981 Addenda.

Specifically, operators placed the Train A spent fuel pool cooling pump in service, with
j
Train B out of service, without obtaining a new set of reference values, af ter performing
j
operability analyses for low system flow six months earlier. This violation is of concern
'
because of other receat issues relating to implementation of your inservice test program.

These violations are cited in the enclosed Notice of Violation (Notice), and the
circumstances surrounding the violations are described in detailin the enclosed report.
Please note that you are required to respond to this letter and should follow the
instructions specifieu in the enclosed Notice when preparing your response. The NRC will
