Interim 10CFR50.55(e) Report: Service Water System Spool Linings, Pipe Inserts and Valve Liner / Seats (CDR 85-00-13)

Dear Sir:

On August 20, 1985, we reported a potential 10CFR50.55(e) deficiency

[ Reference (b)], regarding the failure of Service Water System spool linings, piping inserts and valve liner / seats. This deficiency has been evaluated and has been determined reportable under the provisions of 10CFR50.55(e).

Description of Deficiency During startup testing of the Service Water System, it was discovered that:

o Some polyurethene linings in pipe spools downstream of throttling valves had become detached.

o Some valve liners and seats were found cracked or bubbled.

o Polyurethene insert from the flanged end of the cement-lined pipe, adjacent to a safety-related valve, detached and wrapped around the butterfly disc preventing the valve from closing.

~ Attention: Mr. Richard W. Starostecki Page 2 Safety Implications These deficiencies could cause the flow in either cooling train served by the cooling tower to become degraded and inhibit the function of the system.

Corrective Action An investigation of the failure has begun, and corrective actions have been reconnended. Materials have been selected which we believe are adequate to correct the problem. Repairs in representative locations have begun, and the results will be evaluated during subsequent system operations.

He will report further on this item by January - 15,1986.
