NRC INSPECTION REPORT 50-456/96014; 50-457/96014

NOTICE OF VIOLATION

Dear Mr. Stanley:

On October 18. 1996, the NRC completed an inspection at your Braidwood 1 & 2

reactor facilities. The enclosed report presents the results of that

inspection.

Overall, we found that plant operations were conducted safely.

We are concerned about two incidents of poor performance by non-licensed

operators that resulted in equipment damage to the 1C condensate booster pump

i

and spraying about 150 gallons of water into the diesel generator fuel oil

!

tank room.

These events indicated a failure to properly control evolutions in

i

the field. We are also concerned that NRC inspectors identified that the

l

latest revision of a surveillance procedure was not used when performing the

surveillance on the 1B Diesel Generator which was a violation of NRC

requirements.

The violation is cited in the enclosed Notice of Violation, and the

circumstances surrounding the violation is described in detail in the enclosed

report.

Please note that you are required to respond to this letter and

should follow the instructions specified :.1 the enclosed Notice when preparing

your response.

The NRC will use your response, in part, to determine whether

further enforcement action is necessary to ensure compliance with regulatory

requirements.

With respect to your temporary alteration process, while we recognize that you

have made some recent progress in reducing the number of temporary alterations

in use at the plant we are concerned with the high number that have been in

place for greater than 18 months.

Management controls to ensure that non-

conforming plant problems are permanently fixed in a timely manner have not

been effective in these instances.
