(1) The source was packaged and shipped by F.X. Masse Associates to Los Alamos.
]
Enclosed is a copy of the executed DOE /NRC Form 741 Nuclear Material Transfer Report, documenting the completed transfer.
(2) Enclosed is a completed NRC Form 314 documenting completion of all work under this license.
(3) The area in which the SNM source was used is also routinely used for byproduct material work under our byproduct materials license. Although a thorough survey was performed by F.X. Masse Associates after removal of the SNM source, to assure l
no residual SNM contamination was present, we are not closing out that laboratory or releasing it for unrestricted use.
We trust this additional information will enable the termination of this license. Please do not hesitate to cmarontact Frank Masse at 617-253-9217 if further information is required.

Dear Dr. Peterman:
This is in reference to your letter dated October 10,1996 which requests the termination of License No. SNM-416. In order to continue our review, we need the following additionalinformation:
1.
Included with your letter is a copy of the DOE authorization tc, ship / relinquish ownership of the Mound Laboratory Model M936 PuBe source listed on your license. This form appears to authorize the shipment of the source, but does not provide information regarding the disposal of the material. Please provide additional information regarding who handled the disposal, the date when the source was disposed, and how it was done.
2.
Enclosed is an NRC Form 314 which needs to be completed and returned to Region I. In addition, please provide copies of the shipping and receiving papers regarding the disposal. Include a letter from Los Alamos National Laboratory stating when the shipment of the source was received.
3.
If the area where the Mound Laboratory Model M936 was stored is to be released for unrestricted use, in other words, no other licensed material will be used in that area, a closecut survey must be conducted, and the results submitted to Region i for review before the facility can be released for unrestricted use. Enclosed is a copy of the " Guidelines for Decommissioning Facilities and Equipment Prior to Release for Unrestricted Use or Termination of Licenses for Byproduct, Source or Special Nuclear Material," dated April 1993.
We will continue our review upon receipt of this information. Please reply in duolicate to my attention at the Region i Office and refer to Mail Control No. 123793. If you have any technical questions regarding this deficiency letter, please call James Bondick at (610) 337-6951.
