CATAWBA NUCLEAR STATION REPORT NUMBER: SD 414/84-22 REPORT DATE:
January 31, 1985 FACILITY: Catawba Nuclear Station Unit 2 IDENTIFICATION OF DEFICIENCY:
The operator on Valve 2KC228B was-found to be damaged during I & E checkout of the valve and reported on NCI 19011. Af ter investigation it was determined that the operator (S/N B8243/Al) was the Rotork VNO wiring design. The Duke external wiring for the valve was set-up for the Rotork NO wiring design.
INITIAL REPORT:
On October 25, 1984, A. L. Ignatonis, NRC Region II, Atlanta, G A was notified of potential deficiency by L. M. Coggins, R. L. Oakley, J. H. Henkel and T. L. Utterback of Duke Power Company, Charlotte, NC 28242.

The following paragraph describes the root cause of the deficiency for 2KC228B and several other Unit 2 operators which were discovered in an audit of operator design and external wiring. Rotork made a modification to internal wiring of their operator not obvious to downstream users of the operator and attendant design details were not adequately communicated to users, i.e. valve manufacturers and Duke Power Company. The Rotork NO design incorporated a mechanical latch to lockout the torque switches to unseat valve with full torque and to prevent hammering. The VNO design makes use of an electrical latch which must be furnished by Duke in external wiring. Using the VNO design operator in a NO external wiring design can result in operator damage due to hammering.
Both valve manufacturers and Duke who ordered spare operators and spare switch mechanisms failed in some cases to adequately incorporate the modified operator and its attendant design documents into station design, installation and main-tenance requirements such that several installed operators were incorrectly wired.
o ANALYSIS OF SAFETY IMPLICATIONS:
, The use of VNO design operators in NO design external wiring application can result in damage causing a loss of valve / operator function.
The following is a list of active valve tag numbers discovered in the Unit 2 audit to have incorrect wiring.

2ND001B 2NV253B Two VX System damper operators 2-A R F-D-2 2-A R F-D-4 CORRECTIVE ACTION AND STATUS:
An audit of all Unit 2 active valve operators has been completed and resulted in twelve incorrectly wired operators including 2KC228B.
A controlled list of valve tag numbers and their respective operator serial numbers has been entered into the manufacturer's drawing file. This list will be maintained by Design Engineering.
I Construction's procedures will be revised to require a Variation Notice to be written against this list anytime an operator is replaced or a switch mecha-nism is changed from a NO to a VNO type. At the time the Variation Notice is received in Design Engineering the new operator will be reviewed for correct wiring diagram and electrical external wiring. New Rotork motor operated valve applications will be added to the list as they arise.
The wiring for the eleven operators (excluding 2KC228B) will be complete by June 1,1985. Design Change Authorization CN-2-E1624 has been issued to correct the wiring. Wiring for 2KC228B was corrected by resolution of NCI 19011 and Design-requested VN 47378.
An audit of Unit I has also been conducted. Five valve and two damper operators were wired incorrectly. The corrective action for the five valve operators was completed December 30, 1984 by Nuclear Station Modifications 10496, 10510 and 10512. A Station Problem Report has been issued to correct the damper operators.
Rotork,'Inc. has been notified of this deficiency.
