SUBJECT: NRC INSPECTION REPORT 50-298/96-16
Dear Mr. Horn:
An NRC inspection was conducted December 2-12,1996, at your Cooper Nuclear Station
reactor facility. The enclosed report presents the scope and results of that inspection.
The purpose of this inspection was primarily to evaluate the effectiveness of the licensed
operator requalification program. Overall, the licensed operator requalification training
program effectively implemented a systems approach to training and maintained the
condition of licenses. However, the inspectors identified recurring performance
deficiencies in crew command, control, communication, and procedure usage skills.
Therefore, your corrective actions do not appear to have been effective for all crews to
maintain operator performance at expected levels so that previously identified performance
deficiencies and weaknesses do not reappear.
We are concerned with the recurrence of these performance deficiencies because deficient
crew command, control, communication, and procedure usage skills contributed to the rod
mispositioning event of January 7,1996, pursuant to which we issued a Notice of
Violation (EA 96-202) on September 30,1996. You are requested to respond to this letter
within 30 days regarding both the recurring performance deficiencies and the actions taken
to address individual performance deficiencies as discussed in Section 05.4b in the report.
The NRC will consider your response in determining whether further inspection is
necessary.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,
its enclosure (s), and your response will be placed in the NRC Public Document
Room (PDR). To the extent possible, your response should not include any personal
privacy, proprietary, or safeguards information so that it can be placed in the PDR without
redaction.
