F Appendix NOTICE OF VIOLATION Dairyland Power Cooperative Docket No. 50-409 As a result of the inspection conducted on September 16-20, 1985, and in accordance with the General Policy and Procedures for NRC Enforcement Actions (10 CFR Part 2, Appendix C), the following violation was identified:

10 CFR 20.401 states that the licensee shall maintain records showing the radiation exposures of all individuals for whom personnel monitoring is required under 10 CFR 20.202, and that such records shall be kept on Form NRC-5 in accordance with the instructions or on clear and legible records containing all the information required by Form NRC-5.

Contrary to the above, official radiation exposure records maintained for mechanical maintenance personnel, and some others, during the period March 1984

-through the present are not in accordance with NRC-5 instructions in that lifetime exposure and some quarterly and year-to-date recorded exposures we e in error.

This is a Severity Level IV violation (Supplement IV).

The inspection showed that action had been taken to correct the identified item of noncompliance and to present recurrence. Consequently, no reply to this

-item of noncompliance is required and we have no further questions regarding this matter.
