SUBJECT: NRC INSPECTION REPORT 50-254/96014(DRP), 50-265/96014(DRP) AND

+ NOTICE OF VIOLATION

Dear Mr. Kraft
:

. On October 26,1996, the NRC completed an inspection at your Quad Cities Nuclear r

Power Station facilities. The enclosed report presents the results of that inspection.

'

I Based on the inspectors' assessment during the report period, the overall performance of  !

l the Quad Cities facilitu was safe. The inspectors identified examples of good control panel ,

I monitoring; good maintenance planning and work practices on safety-related equipment: '

and a thorough root cause evaluation concerning problems with the high pressure coolant

injection system. However, the inspectors noted severalinstances of poor supervisory

oversight for work on important safety systems. In one cass', unqualified workers on

backshift were working on the Standby Diesel Generator (SBDG), a risk significant safety

i

system, which was out of service in a 7 day limiting condition for operation, with less than

1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> of supervisory oversight during the whole shift. Also of concern were poor quality

parts which were not detected by the receipt inspection process.

The inspectors identified two violations of regulations. One violation resulted from two

examples of failure to follow station procedures in the maintenance and operations areas. 1

The other violation involved failure of the Quality Assurance and Maintenance programs to ,

ensure individuals were qualified to perform maintenance activities on a SBDG.

The examples of failure to follow station procedures were of concern because:

1) maintenance procedures on a SBDG not being followed led to rework and incorrect

work practices in some cases, and 2) having an unapproved version of a posted graph had

the potential for providing incorrect information to control room operators. Additionally,

corrective actions for a similar concern with unapproved plant postings were not effective. /j

The Quality Assurance and Maintenance program violation is of concern because

qualifications and training of workers involved with safety-related activities were not being

assured.

you are required to respond to this letter and should follow the instructions specified in the i

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter

and its enclosures will be placed in the NRC Public Document Room (PDR).
