SUBJECT:
LICENSEE TROUBLESHOOTING PLANS TO DETERMINE EQUIPMENT MALFUNCTION CAUSES Attached are three equipment troubleshooting plans obtained from the Davis-Besse licensee:
- 1) Auxiliary Feed Pumps Overspeed Trips
- 2) Review of the Operation of the PORV
- 3) Auxiliary Feedwater System Valve Problem Analysis (AF 599 & 608)
These plans discuss the hypotheses identified to date by the licensee for the causes of equipment problems which occurred during the June 9,1985 event. These are being forwarded to you for your consideration in any generic communications to be issued on the event.
It should be noted that the fact finding team has not at this time reached any definite conclusions on root causes for the various equipment problems.
Conclusions will not be reached until the licensee has completed the trouble-shooting and the team has reviewed the licensee's test results and analyses for identifying root causes.
