Dear Sir:

In Reference (d) we issued the final report regarding the completion of all activities taken to correct the cracked concrete pedestals supporting the Diesel Generator Exhaust Silencer Tank.

Subsequent verification by a NRC Resident Inspector determined that all of the required rework had not been completed.

Investigation revealed n

that the sign off of the work release by a QC inspector was incomplete as og the inspector interpreted that he was signing off only for the welding ago.

that he had inspected. The verification by QA that the work was completed consisted of a documentation review only.

It is normally required, when

$o Possible, that the hardware be verified for work completed as well as the documentation verification. The hardware verification was not accomplished in this case.

A nonconformance report was intiated and the required work has now s

been completed and reinspected.

Concurrently, the responsible contractor initiated a Corrective Action Request (CAR) which requires a documented review of other potential problem areas in his area of responsibility. Also, specific retraining of all applicable QC/QA personnel in the handling of work requests will reinforce the importance of maintaining control and traceability of work packages.
