Dear Mr. D.glish:

The City of Stillwater, Oklahcrna in 1979 began application process to obtain a license frcm the !Melear Regulatory Ccnmission in order to reintain and use a nuclear density nachine in order to inprove subgrades we were getting under our paving.

City Staff wr.s trained in the use of this machine and in the hazards of nuclear radiation. We were all certified to opsrate the equignent and nede application to the !belear Regulatory Corrmission in Decwber of 1979 to becme licensed in order for us to purchase a nachine.

The City of Stillwater kr finally licensed on February 2f,,1980, so that we could obtain ano y ate a density nachine. Since tLt time, .

we have been filing our re eports with the IMclear Regulatory Ccrimission. Recently we N w a snowed with letters frcm Mr. Stello concerning overdue personnel nonitoring reports for the years 1978 cnd 1979. These letters have been sent out on form letter fortrat with no specific addressee. Our reaction has teen to file these in the kuste basket because we were not even licensed in 1978 and'1979. They con-tinued to send us these reports even threatening to take away our license if W did not respond. 'Iherefore, after nany man hours of time wasted on tMelear Regulatory acrrmission paperwork, we have finally checked personnel nonitoring not required in the years requested and nailed the paper work back.

HEMORANDUM FOR: NRC Licensees  ;

FROM: Victor Stello, Jr., Director Office of Inspection & Enforcement Sth. OVERDVE PERSONNEL MON $TORING REPORTS Amendments to 120.407,10 CFR Part 20, published in the FEDERAL REGISTER on September 29,1978 (43 FR 44827) required all NRC specific licensees to report, the results of personnel monitoring for each of the in statistical years 1978 andsumary 1979. The form,rep ' orts were due March 31 in 1979 and 1980, respec-tively. Copies of the amendments were sent to all NRC licensees by letter dated October 10,1978(copyenclosed). Subsequent reminder letters were sent on January 29, 1979 and November 30, 1979.

As of this date es have not received a report of your monitoring results for 1979 that can be identified with the NRC icensenumber(s)thatappear(s)intheupper corner of the address label on this letter. .

Soo of these circumstances are shown on the enclosed " Potential Reasons for Failu,*e to Identify Submitted Reports." If you believe this.to be your case, you shculd advise us of the circumstances involved with enough additional infonnation to cleirly identify the license number (s) and to complete the report.

You may use the enclose 6 suggested format to file your report (s). Note that, if you were not required to provide personnel monitoring equipment to assess any individual's radiation dose during 1979, you only need to check the box on the suggested fonnat and return the format to the Director of Management and Program Analysis, U.S. Nuclear Regulatory Commission.
