SUBJECT: NRC INSPECTION REPORT 50-483/96-09 AND NOTICE OF VIOLATION
Dear Mr. Schnell:
An NRC inspection was conducted September 1 through October 12,1996, at your
Callaway Plant reactor facility. The enclosed report presents the scope and results of that
inspection.
During the 6-week period covered by this inspection period, your conduct of activities at
Callaway was generally characterized by safety-conscious operations; sound engineering
practices; and appropriate performance of maintenance, surveillance, and plant support
activities. We are concerned, however, about one violation of NRC requirements that was
identified by the NRC inspectors. The violation involved failure to properly translate a
modification to a hydrogen recombiner into plant procedures. The violation is of concern
because it indicates a weakness in your design modification process that did not ensure
procedures associated with the modification were revised prior to modification
implementation.
The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding this violation are described in detail in the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
