NRC INTEGRATED INSPECTION REPORT NOS. 50 325/96-13 50-324/96-13
Dear Mr. Campbell:
On September 14 the NRC completed an ins;ectico 4 your Brunswick reactor
facility. The enclosed report presents tie resui s of that inspection.
During the six weeks covered by this inspection period, our ins)ectors found
that your staff generally took a safety conscious approach to t1e activities
conducted at the Brunswick Plant. We are concerned, however, about five
violations of NRC requirements that were identified.
In the first v!alation
an operator repositioned a selector switch while tagged with a hoid order.
This could have caused a personnel hazard or equipment damage.
In the second
violation an operator failed to complete the required training program prior
to taking an audit examination.
In the third violation a new fuel criticality

monitor was not set correctly as required by Federal Regulations.
The fourth and fifth violations included a failure to follow procedure (s) to
control radioactivity and a failure to conduct adequate radiological surveys
commensurate with the potential hazards that were present.
We are concerned
that these issues indicate a lack of attention to detail which could
negatively impact ongoing radiation protection activities. We are also
concerned that three separate NRC inspections since Se)tember of 1992 have
identified survey violations. Corrective actions to t1ese violations do not
appear to have been fully successful over the long term.
The violations are cited in the enclosed Notice of Violation and the
circumstances surrounding the violations are described in detail in the
enclosed report. Please note that you are requirea to respond to this letter
and should follow the instructions specified in the enclosed Notice when
preparing your response. The NRC will use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance
with regulatory requirements.
