This letter is in response to the February 22, 1985 Notice of Violation concerning unacceptable porosity in an ASME Code,Section III weld. Please refer to Attachment A for the details of the response.

l Subj ect : Unacceptable Porosity;in ASME Code,Section III Weld The': Notice of: Violation states in part:-

'...the residual heat removal pressure boundary weld:

RH-13-FW9Q2 was determined to have porosity which exceed:the

~ requirements.of. Appendix-VI of ASME.III and had been erroneously accepted.by the licensee.

-I. Corrective Action Taken and Results Achieved The unacceptable weld was documented on Nonconformance Report (NCR).23649 dated November 10,-1984. The disposition was to rework the weld by grinding. Addition of weld' filler L metal was not required to restore the weld to'ASME Code,Section III, requirements. Subsequent radiography indicated an acceptable weld, and NCR 23649 was closed on November 12, 1984.

Acceptance of the' original weld was by a certified NDE Level II. employed by Baldwin Associates from March 21, 1984 -

through May 18, 1984. Work performed by this individual during this period was reviewed by a certified NDE Level III.. No other unacceptable welds were identified. This review was completed by December 17, 1984.

II. Corrective Action to'be Taken to Avoid Further-Noncompliance The subject individual's work has been reviewed and found to be adequate. No further corrective action is required.

'III. Date When Full Compliance Was Achieved Illinois Power Company was in full compliance by December

17, 1984.
