Also in late September 1990, computer printouts'~were i l obtained of summary reports of inspections of the'18. groups-of I gauge licensees'cVer the past 7 years. The computer printout ,

provided a list of: '

the name i report number when letter form 591 was issued enforcement conference (yes,no) severity level and a description of the findings.

There were approximately 2,729 inspections of general and  ;

specific licensees during that period. Each violation was briefly stated in the report. Many of the licensees were inspected more than once in that period.

J' There is a total of 6,356 reports of inspection for the period. A total of 3,627 (57%) of the reports show no violation of regulatory requirements. The other 2,729 (43%) reports each show one or more violations of regulatory requirements resulting in 6,222 violations (averaging 2.3 violations per report). The most frequent violations are:

1. Failure to comply with transportation regulations (labeling, secure during transport, approved transport packaging, etc.) - 974 or 16%*
2. Failure to perform and record periodic leak tests as required by license condition - 820 or 13%
3. Failure to perform and record periodic inventory as required by license condition - 606 or 10%
4. Lack of personnel monitoring and records of such

- 457 or 7%

5. Failure to post notices to workers as required by 10 CFR 19.11 - 447 or 7%

. 6. Lack of calibration of survey meters and pocket dosimeters and records of such - 256 or 4% ,

7. Unauthorized users - 181 or 3%
