Page 2 of 2 The purpose of this modification is to: (1) increase the level of effort in accordance with the attached Statement of Work. (2) increase the contract ceiling and obligated amounts by $29.904.00 from $351.161.59 to $381.065.59, and (3) extend the period of performance from September 30, 1996 to March 28.

Accordingly, the contract is hereby modified as follows:

Under Subsection B.3. " CONSIDERATION AND OBLIGATION--COST PLUS FIXED FEE (JUN 1988), paragra3hs (a) and (c) are deleted in their entirety and the following is su)stituted in lieu thereof:

"(a) The total estimated cost to the Government for full performance of this contract is $381.065.59, of which the sum of $356.136.70 represents the estimated reimbursable costs, and of which

$24.928.89 represents the fixed fee."

"(c) The amount obligated by the Government with respect to this contract is $381.065.59."

Subsection F 8. " DURATION OF CONTRACT PERIOD (MAR 1987)" is deleted in its entirety and the following is substituted in lieu thereof:

"This contract shall commence on November 29. 1993 and shall expire on March 28, 1997."

All other terms and conditions of this contract remain unchanged.

A summary of obligations from award date, through the date of this action are as follows:

NRC-03-94-034 Modification No. 8 Page 1 of 3 STATEMENT OF WORK The contractor shall perform the following additional work requirements in accordance with Subsection C.4 " Work Requirements and Schedule" of the basic contract.

The effort for the new tasks and the effort for current tasks shall be integrated to provide the most effective method for completing all of the work on or before March 28, 1997.

(a)

Additional Data Entry - The contractor shall make all necessary corrections to the IFS data for NRC Regional Offices Two and Three.

The contractor shall use the data previously generated on the data correction forms to enter corrections into the IFS system for FY1991 through FY1994.

All data entered will receive a separate quality control check for accuracy of entry.

(b) 766 Escalated Enforcements - The contractor shall enter escalated enforcement actions for FY1989 through FY1991 into the 766 system (now referred to as the SES -

Statistical and Enforcement System).

The contractor shall check the existing 766 or SES system to assure that there is no duplicate data and delete any data about these escalated violations that conflicts with the current data.

If the 766 or SES system does not already contain this information/ data, then the information/ data i

shall be entered into the 766 or SES system.

(c)

Create NUREG-like Reports - For each fiscal year previously researched by the contractor (FY1989 through FY1994), the contractor shall prepare a publication similar in format and style to an NRC NUREG publication.

The report shall serve as a final product for the contract and will be delivered in both hard copy and electronic (3.5 inch diskette) formats.

The report shall contain a listing by year and by docket number (i.e.,

power and non-power reactors and vendors) of all of the inspection reports for that docket.

The report will detail the quantity and severity of any violations, deviations, and/or non-cited violations listed for that inspection report.

The information will reflect all of the additions, deletions, and corrections researched by the contractor.

Vendor Inspection Reports -

The contractor shall provide

quality assurance and data correction for Vendor Inspection Reports issued in FY1995, FY1996 and the first five months of FY1997. The contractor shall perform the same analysis process previously used'for the prior

MEETINGS / TRAVEL Travel requirements (local) for the tasks specified in this modification are as stated below:

The contractor shall travel to NRC Headquarters in Rockville, Maryland, to pick up and deliver various NRC documents and reports, and to attend meetings as required.
