SUBJECT: NRC INTEGRATED INSPECTION REPORT

Dear Mr. Mueller:

On July 26, 1996, the NRC completed an inspection at your Zion 1 and 2 reactor

facilities. The enclosed report presents the results of that inspection and a

requalification examination administration inspection that occurred in the

same inspection period. In addition, this report also integrates the results

of en operational performance inspection that was conducted from May 20

through 24, 1996.

During the five week period covered by this inspection, your conduct of

activities at the Zion facilities indicated a continuing trend of inattention

to detail by plant personnel. Inspectors identified that failure to follow

procedures had led to misoperation of the 2B emergency diesel generator and

licensee personnel demonstrated a lack of understanding of the operation of

the CO, portion of the fire protection system. Inspectors identified that

improper credit had been granted for maintaining active operator licenses.

Also, inspectors identified that material removed from a contaminated area was

not surveyed and packaged properly to prevent the spread of contamination.

Finally, inspectors identified that prompt corrective action was not taken to

address and correct a puncture hole in the exhaust ventilation duct from the

holdup tank room that created a potential inleakage point that was not routed

through charcoal filters.

These violations are cited in the enclosed Notice of Violation, and the

circumstances surrounding the violations are described in detail in the

enclosed report. Please note that you are required to respond to this letter

and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to

determine whether further enforcement action is necessary to ensure compliance

with regulatory requirements.
