SUBJECT: NRC INSPECTION REPORT NOS. 50-373/96007(DRP) & 50-374/96007(DRP)
Dear Mr. Subalusky:
From June 23 through August 2, 1996, the NRC performed an integrated team
inspection at your LaSalle facility. The enclosed report presents the results
of that inspection.

Based on the results of this inspection, the NRC has determined that three
violations of NRC requirements occurred. These violations are cited in the
enclosed Notice of Violation (Notice) and the circumstances surrounding them
are described in detail in the subject inspection report.
)
One violation involved an event in June 1996 which illustrated that previous
corrective actions had not been fully effective. The event involved a main
]
steam isolation valve closure and reactor scram.
NRC is concerned that a
similar event occurred in 1994, and a problem identification form initiated in
March 1995 outlined a procedural weakness which contributed to the most recent
)
event; however, ne'ther resulted in broad enough corrective actions to prevent
the June event. ihe two other violations occurred in the fire protection area
and in conduct of Technical Specification surveillances. The consequences of
these events were less significant.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence.

include previous docketed correspondence, if the correspondence adequately

addresses the required response. After reviewing your response to this
Notice, including your proposed corrective actions ed the results of future
inspections, the NRC will determine whether further Nk0 enforcement action is
