Appendix NOTICE OF VIOLATION Dairyland Power Cooperative Docket No. 50-409 As a result of a licensee notification to the NRC and a followup inspection by Region III inspectors, and in accordance with the General Policy and Procedures for NRC Enforcement Actions (10 CFR Part 2, Appendix C), the following violation was identified:
The LACBWR Technical Specifications, paragraph 4.2.2.18, require that the low pressure coolant injection system shall be available for automatic operation except at times when the reactor is shut down and the systen depressurized to approximately atmospheric pressure. Manual valves in this system shall be locked in that position which will not impair system capability when automatic operability is required.
Contrary to the above, on April 13, 1985, with the reactor plant shut down but pressurized to perform a hydrostatic pressure test, the alternate core spray system, which is part of the low pressure injection system, could not have performed its safety function if called upon to do so because two manual valves in the system were closed and locked out.
This is a Severity Level IV violation (Supplement 1).
The inspection, and subsequent review of LER 85-07, showed that action was taken to correct the violation and to prevent recurrence. Therefore, no reply to this Notice is required.
