SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-010/96009, 50-237/96009,

AND 50-249/96009 AND NOTICE OF VIOLATION

Dear Mr. Perry:

This refers to tne inspection conducted on July 13 through August 30, 1996, at

the Dresden Nuclear facility. The purpose of the inspection was to determine

rhether activities authorized by the license were conducted safely and in

accordance with Nuclear Regulatory Commission (NRC) requirements. During this

period, routine resident and special inspections of radiation protection and

control of licensed operator qualifications were performed. In addition, an

assessment of your readiness to implement the Technical Specification Upgrade

Program (TSUP) was conducted. At the conclusion of the inspection, the

findings were discussed with those members of your staff identified in the

enclosed report.

Overall, during this reporting period, some specific items were identified

which indicate a lack of attention to detail and has resulted in a decline in

performance in some areas. This issue warrants continued management attention

and oversight. Of particular concern was returning an individual to licensed

duties without having that license properly reactivated.

Five violations were identified during the inspection period. The first

violation concerned the failure to perform an on-site review of a special

procedure for testing electrical . Bus 33-1. The second regarded returning an

individual to licensed duties without taking the actions necessary to fully

reactivate that license. The third concerned failure to take effective

corrective actions with two examples. The fourth involv:i poor radiation

worker practices that resulted in an unnecessary exposure. The fifth

regarded a Technical Specification violation when all Unit 3 diesels were

inoperable due to 4kV bus work and diesel surveillance scheduling. These

violations are cited in the enclosed Notice of Violation (Notice), and the

circumstances surrounding the violations are described in detail in the

enclosed report. Please note that you are required to respond to this

letter and should follow the instructions specified in the enclosed
