Systematic Assessment of Licensee Performance (SALP) and Mr. W. P Murphy's

Letter, FVY 85-22, March 1, 1985

This refers to the SALP for the Vermont Yankee Nuclear Power Station, conducted

by_this office on December 11, 1984 and discussed with you and your staff at a

meeting on January 25, 1985. The list of meeting attendees is attached as Enclo-

sure 1.

The NRC Region I SALP Report as amended is provided as Enclosure 2 and

covers the period May 1, 1983 through October 31, 1984.

Mr. Murphy's response

date.d March -1,1985, submitted pursuant to our letter of December 21, 1984, pro-

vided comments on the SALP Report. These letters are provided as Enclosures 3

and 4.

Our overall assessment of your performance of NRC licensed activities at the Ver-

mont Yankee facility is that there is a generally conservative approach taken to

assure safety in plant operations.

Additional strengths in your management con-

trols were_found in the planning and control of outage activities and design

changes, implementation of effective housekeeping and security programs, completion

of licensing actio1s, and in your preventive maintenance and operational surveil-

lance programs.

Significant improvements were evident in the emergency prepared-

ness area.

,

Some performance shortcomings were observed, however, in the areas of radiation

protection, fire protection and maintenance.

There were numerous personnel errors

noted during the performance of routine operations.

Increased management attention

is warranted to assure the personnel error trend does not continue.

Additionally,

the policy for_ operation under apparent anomalous conditions should be reviewed

and strengthened as necessary to assure that conservatism in plant operations is

maintained at previously observed levels.

Based on our discussions, continued NRC

and Vermont Yankee management attention will be directed to these areas to assure

identified weaknesses have been corrected.

In the meeting of January 24, 1985, we discussed our assessment of your regulatory

performance and received your comments on the SALP Program and our assessment.

We have considered your letter of March 1, 1985 and found that there are some dif-

ferences of opinion.

Accordingly, we are providing you our detailed evaluation

in Enclosure 5.

Our SALP conclusions have not been effected.

However, for the

purposes of clarification of the SALP report, we have added the fifth sentence on

Page 26.

In response to your letter comment Item (2) on the Appendix R violation,

we do not agree with your comments in general and, in particular, with your posi-

tion that your response to the findings has been timely.
