DATE OF ISSUE: DECEMBER 21, 1992 This is an addenda to the loose-leaf version of the Inservice Testing Program, Revision 6, and is issued in the form of replacement or cdditional pages which contain changes, additions, or deletions.
SUMMARY
OF CHANGES This is the third addenda to be published to the Inservice Testing Program, Revision 6.
Changes listed below are identified on the affected program pages by a margin note (i.e., 12/92) adjacent to the affected area, with the exception of pump or valve summary listing pages.

26 Added a "CTCP" to the TEST RQMT column and-a "Q" to the TEST FREQ column for valve CIC's RHR-CV-10CV, RHR-CV-11CV, RHR-CV-12CV, and RHR-CV-13CV. A partial stroke test is required for these valves.
32 Added a "CTCP" to the TEST RQMT column and a "Q" to the TEST FREQ column for valve CIC HPCI-CV-13CV.

Hand held tachotutors have proven to be more reliable instruments as far as producing consihtent readings.
Tr e tachomaters vailable to be used for IST, have ranges of 10 99,999 rpm or' 6 30,000 rpm.
The respective reference value is 4500 rptn.
Therefore, the range limitation irnposed by IWP 4120 (3 timos the reference value or less) is not met by those hand held
-r tachometers.
However, the accuracy of the 10-99,999 rpm tachorneter is i 9 rpm, i
An instrument accuraay of i 9 rpm on a full range of 0 13,500 rpm (3 x 4500 rpm) wout be i.07% of the full range which is better than the accuracy required by IWP-4110 (i.e.12% of full scalc)'.
For the 6-30,000 rpm tachometer, accuracy is i 99 rpm.. i 99 rpm accuracy on a range of 0 13,500 rpm is 1.73%. which is also better than i 2%.
Therefore, the proposed range and accuracy results in measurement with accuracy better than Code. requirements and will - provide reasonable assurance of component operational readiness.

A different valve will be disassembled, inspected and manually exercised during each refueling outage until the entire group has been tested.
If the disassembled valve is not capable of being full-stroke exercised or thete is binding or failure of valve internals, the remaining vwives in that group will also be disassembled, inspected and manually exercised during the s t.me outage.
In addition, a partial stroke exercise test, with fl ow, 12/12 will be per f orroed quarterly, and after each disassembly / inspection.
