Gentlemen:

SUBJECT: NRC INSPECTION REPORT 50-498/93-01; 50-499/93-01 (NOTICE _0F

VIOLATION)

This refers to the inspection conducted by Mr. T. O. McKernon, and the

personnel identified in the enclosed report, of this office on January 11-15,

1993. The inspection included a review of activities authorized for your

South Texas Project, Units 1 and 2, facility.

A'. the conclusion of the

inspection, the findings were discussed with you and those members of your

staff' identified in the enclosed report.

Areas examined during the inspection included an assessment of your licensed

operator requalification program. Within this area, the inspection consisted

of selective examination of procedures and representative records, interviews

with personnel, and observation of activities in progress.

Based on the results of this inspection, certain licensed activities appeared

to be in violation of NRC requirements, as specified iei the enclosed Notice of

Violation (Notice). The violation is of concern because the failure to have a

formal approved training plan is inconsistent with a systems approach to

trainin9

It was also identified that an individual entered the radiological control

area without the prerequisite dosimetry and this was in violation of your

Technical Specifications, Section 6.11 for failure to follow-procedures. This

violation is not being cited because the criteria in paragraph VII.B.2 of

Appendix C to 10 CFR 2 of the NRC's " Rules of Practice," were satisfied. This

violation was an isolated occurrence and the licensee staff took prompt and

effective actions to correct the problem.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence.
