Gentlemen:

Subject: Special Inspection 50-213/85-15 (June 14-26, 1985)

This inspection of the Haddam Neck Plant reviewed the circumstances and corrective

actions related to your identification of design change control deficiencies in

Plant Design Change Request (PDCR) No. 713, Process Computer Replacement, and to

the potential inoperability of control room fire protection equipment caused by

this ongoing modification. The findings were discussed with Messrs. E. Mroczka

and R. Graves of your staff.

The inspection identified Violations related to the design review and safety

evaluation for the process computer replacement project, and to the onsite review

committee review of these modifications. Please reply to these matters in accor-

dance with the enclosed Appendix A.

We note that inadequate attention to detail at several levels of the aesign review

process appears to have been a factor in the violations cited in this inspection.

We also note that your corrective action after identification of design review

process failures in the process computer modifications may not have been prompt

enough to prevent recurrence of similar violations and that your response to the

reported inoperability of three control room smoke detectors failed to promptly

restore those detectors to a fully operable condition. We fully recognize that

your subsequent evaluation concluded that your original classification of detector

inoperability was conservative, and that the control room fire detection system

actually remained operable. Nevertheless, your actions based on the best informa-

tion available at the time could have resulted in a needlessly prolonged period

of system inoperability. Consequently, in addition to your reply to the violations

noted above, please include your evaluation of the effectiveness of your corrective

action programs in these instances and any further measures you have planned or

completed to improve your performance in this regard.
