SUBJECT:

AIT CHARTER - V0GTLE Enclosed for your implementation is the Charter for the inspection of events associated with the Vogtle Unit 1 Loss of Off-site Power Event on March 20, 1990. This Charter, prepared in accordance with the NRC Incident Investigation Manual, dated February 1988 Manual Chapter 0325, reflects the needs of Region 11, NRR, and AEOD management. The objectives of the AIT are to communicate the f acts surrounding this event to - Regional and Headquarters management, to identify and comunicate any generic safety concerns related to this event to Regional and Headquarters management, and to document the findings and conclusions of the onsite inspection.

ENCLOSURE V0GTLE AUGMENTED INSPECTION TEAM CHARTER

1. Evaluate the activities and plans which ' established the initial plant '

conditions. Identify the initial plant conditions (prior. to start of transient).

2. Develop and validate a detailed sequence of events associated with 'the loss of all AC power transient on Unit 1.
3. Identify any procedural requirements and/or deficiencies associated with the fuel truck's movement in the protected area. Evaluate the potential for a fuel detonation in this scenario and whether Fitness for Duty standards were complied with.

4 Develop ar.d validate a detailed sequence of events associateo with the implementatiten of the Emergency Plan Implementing Procedures to include the' problems associated with the ENN and ERF. computer.

5. Evaluate the vulnerability of Unit 2 to a Unit 1 switchyard malfunction, .

and vice versa. Determine whether the Unit I switchyard breaker actuations were appropriate and expected.

h 6. Evaluate the response of the '1A' Diesel Generator to the transient, to include equipment perfonnance (Blackout Sequencer and - Jacket Water Pressure).

7. Evaluate the performance of the control room operators and other key plant' personnel who responded to the event, to include- at -least RCS heatup and potential containment challenges. Conduct interviews as necessary to ascertain both strengths: and weaknesses, on personnel training, adequacy of procedures, management control. and communications during the event.
8. Prepare a special inspection report documenting the results of the abo.'

INCIDENT INVESTIGATION TEAM ARRIVES AT YOGTLE SITE An Incident Investigation Team (IIT) is at the Vogtle site to investigatt * % event of March 20, 1990, which resulted in a Site Area Emersancy when Unit 1 lost offsite and onsite AC power to both vital busses. An Augmented Inspection Team (AIT) has been at the site since March 22, 1990. However, due to the number of past incidents-which have occurred at plants while in shutdown conditions and the potential for regulatory concerns, the agency has decided this most recent event, at Vogtle, warrants the more formal and detailed review of an IIT.

The team is composed of membert from AE0D NRR. Region I, and Region V. Under the provisions of a Memorandum of Agreement with the Institute of Nuclear Power erations (INPO), there is also industry participation on the team, a member from

( 70 and two consultants.

Currently, Unit 2 is operating at full power; Unit 1 is in refueling mode. The team held an entrance briefing with utility management 9:00 a.m. on March 26 and the team leader participated in a press briefing at 12:00 p.m.
