SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-369/96-07 AND I

50-370/96-07,' NOTICE OF VIOLATION AND NOTICE OF DEVIATION

Dear Mr. McMeekin:

On September 7,1996, the NRC completed an inspection at your McGuire 1 & 2

reactor facilities. The enclosed report presents the results of that inspection.

During the six-week period covered by this inspection period, your conduct of

activities at the McGuire facilities was generally characterized by safety-conscious

,

operations, sound engineering and maintenance practices, and careful radiological i

work controls. However, violations of NRC requirements were identified regarding (1)

the failure to perform technical specification surveillance requirements within the

required time period involving waste gas decay tanks and the emergency diesel

generator; (2) the failure to perform an adequate 50.59 safety evaluation prior to

conducting a test to confirm assumptions of the effects of steam voiding and high

temperatures on auxiliary feedwater piping; and (3) the failure to take adequate

corrective action to preclude a repetitive emergency diesel generator fuel line failure. '

In addition, a Deviation was identified for failure to comply with responses to  ;

commitments for Generic Letter 88-03, Steam Binding of Auxiliary Feedwater Pumps.

'

The Deviation involves the installation of the incorrect type resistance temperature i

detectors resulting in non-conservative temperature readings of auxiliary feedwater

piping and also inadequate compensatory measures taken once the problem was l

identified.

The Violations and Deviation are cited in the enclosed Notice of Violation and Notice  ;

of Deviation (Notices), and the circumstances surrounding the issues are described in

detail in the enclosed report. Please note that you are required to respond to this i

letter and should follow the instructions specified in the enclosed Notices when

preparing your response. The NRC will use your response, in part, to determine

whether further enforcement action is necessary to ensure compliance with regulatory-  ;

requirements.
