NRC INTEGRATED INSPECTION REPORT 50-456/96011; 50-457/96011
Dear Mr. Stanley:
On July 26, 1996, the NRC completed an inspection at your Braidwood 1 & 2
reactor facilities. The enclosed report presents the results of that
inspection.
During the 6-week period covered by this inspection period, the inspectors
found problems with procedure adherence and communications between workers.
Failure to follow important procedures was demonstrated during a Unit 1
containment release, restoration of an electrical lineup on Unit 2, and
,
control of a temporary alteration to the radioactive waste building
'
ventilation system. The failure to follow procedures during the containment
release and the electrical system restoration were due to poor communications
between operators.
In addition, degraded performance of a radiation monitor
was noticed by a health physicist and this information was not communicated to
the operations department for ten days.
Procedural adherence probicins were
previously discussed in Inspection Reports 96006 and 95017.
Communications
problems between workers have been previously discussed in Inspection Reports
96009, 96008, and 96006.

The failure to properly restore the electrical lineup and the failure to
control the temporary alteration to the radioactive waste building ventilation
system are cited in the enclosed Notice of Violation, and the circumstances
surrounding the violation are described in detail in the enclosed report.
Please note that you are required to respond to this letter and should follow
the instructions specified in the enclosed Notice when preparing your
response. The NRC will use your response, in part, to determine whether
further enforcement action is necessary to ensure compliance with regulatory
requirements.
