Appendix Notice of Deviation As a result of the inspection conducted June 4-7 and fune 12, 1985, it appears that certain of your activities deviate from connitments made to the NRC in your letter dated July 31, 1981, from C. W. Fay to H. R. Denton.
Specifically, commitments were made to accomplish and document the (1) evaluation of personnel to meet minimum qualification requirements, (2) periodic evaluation of inspectors' work performance to ensure a continued high level of qualification, and (3) demonstration of an inspector's proficiency before he is permitted to perform independent inspection.
During this inspection, no objective evidence was identified to support the accomplishment of these connitments.
Please provide in writing within 30 days of the date of this Notice a description of corrective actions taken regarding this deviation, actions taken to avoid further deviations, and the dates when these actions were or will be completed.
