Subject: NRC Con.bined Inspection Mos. 50-245/92-26; 50-336/92-28;

50-423/92-22

An announced safety inspection of the radiological controls, radioactive waste handling, and

radioactive waste transportation programs at the hiillstone Nuclear Power Station was conducted

by hiessrs J. D. Noggle and R. L. Nimitz on October 19-23 and November 2-6,1992. The

inspection findings were discussed with members of your staff on October 23 and November 6,

1992.

Areas reviewed during the inspection were important to health and safety and are discussed in

the enclosed inspection report. These areas included review of: action on previous findings; _

audits; organization and staffing; solid radwaste waste streams; transportation and solid

radwaste; steam generator shipment preparation; radioactive and contaminated material, and ,

contamination controls; radiological controls; and station housekeeping.

The inspector's review identified very good efforts on previously identified matters.

Improvement in the quality of audits of the radiological controls program was noted. The

radwaste and radiological controls organizations were found to be adequately staffed. Good

control of radioactive and contaminated material, and contamination was identified bat two

issues, associated with identification of a contaminated ga-ge in a warehouse and lack of defined

alarm set point for site egress portal monitors were identified as issues needing further review

by your staff. The circumstances surrounding the release of the contaminated gauge is

considered an unresolved item. Preparation and planning for the shipment of the No. 2 steam

generator were good. The radiological controls program was considered overall to be effective

in reducing aggregate radiation exposure of personnel. Housekeeping was considered good,

Two violations were identified. The first violation involved failure to collect representative

samples of Unit 2 resins for determination of curie content for shipping purposes. The violation

is presented in Enclosure 1, Notice of Violation (Notice). You are required to respord to the

Notice and should follow the instructions provided therein. The second violation involved

failure to post certain liigh Radiation Areas inside the Unit 2 containment. Because other signs

and postings, which provided workers appropriate radiological controls information, were posted

at the areas, and appropriate corrective actions were taken to address this matter, the violation

was considered of minor safety significance and in accordance with the Enforcement Policy

(Section Vll.B.1) is not being cited.
