Power was to 60% for the equivalent of about eight hours on October 1, on October 9, and on October 24 to; enter the heater bay for mah69nanco and testbeg. Except for other minor reductione to perform.CRD exerciseo, eve /eillane 5,

On October 19, 1992, the plant entered a 30 day administrative limiting condition for operation (LCO) for maintenance on the out of. service"A" control Building'
. Chiller. The LCO was required by a plant special order and c erating instructions, but not by the technical specifications.- The "B" Control Building Chiller was in i
service to supply chilled water for the cooling coils of the t.ssociated air conditioning unit.
The chilled water system and air conditioning units maintain.
acceptable temperature for control room plant personnel equipment.
On October 22 at 12:5G hours,.with the plant at 99.9%, thermal power and the "A"
chiller still out-of cervice, the "B" chiller tripped. Attempts to re-start the chiller were successful, but.it tripped again, after a short period of cime, for
-unknown reasons. At 13:21 the "B". chiller was declared inoperable since operation could not be maintained, and a.72 ndministrative hour LCO was enter >d.
As part of trouble shooting ectivities,-maintenance personnel discovered air in the chilled water. piping which they vented. The "B" chiller was :estarted at 15:10. At 16:10 the
- "B" chlller was declared operable and the 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> LCO was exited. The 30 day LCO-for the "A" chiller. remained in effect.
LER 92 - 016 (pending) l-On October 26, witn the reactor operating at2100%, a quality assurance surveillance identified four missed hourly firewatches, two in-the Residual Heat Removal la
'e Room on October 10, and two in thc High Pressure Coolant-Injection Room on October 7 arJ 10.
The personnel who performed the-firewatches failed to pro prly'use the firewatch checklist.
All personnel who perform firewatches have slnce been instructed on the'use of-and adherence to the firewatch checklist. All-firewatch perscnnel will have additional formal training on the proper firewatch methods and checkliet. usage.
Fire-detection and suppression e@ ipment in the affected areas was operable throughout the evert.
The Event had no effect'on the safe continued operation of;the facility. LER 92-017-(pending).
